I agree to all conditions of this agreement and hereby accept possession of said animal at my own risk and release Windmill Animal Rescue (W.A.R.) from all liability which I may have now or in the future for any damages to person or property caused by said animal(s). Breach of Contract: Adopter agrees to pay Windmill Animal Rescue the adoption fee as liquidated damages in the event the terms of this contract are breached as well as all attorneys’ fees and court costs that are incurred enforcing this Agreement. This liquidated damage value is agreed to for the purpose of establishing value of the animal and does not bar Windmill Animal Rescue from seeking return of the animal by a judicial process or other legal means. I agree that if I breach the terms of this agreement, ownership of the cat will immediately revert to W.A.R which will have the right to reclaim possession of the cat. Refund Policy: Unless arrangements are made prior to adoptions, all adoption fees are non-refundable and non-transferable. Adoption Fee Includes: Spay/Neuter, Microchip, Initial Immunizations, Tests/Other (refer to records). Adoption Fee: $200 +tax for one cat and $350 +tax for two cats.

- DO NOT DECLAW AGREEMENT: I understand that this is a painful and unnecessary mutilation that could result in chronic paw pain, infection, inability to walk properly and other physical ailments, and the inability for the cat to protect itself, as well as psychological problems that could result in the cat exhibiting behavior such as urinating outside the litter box, biting, fearfulness, anti-sociability and aggressiveness. Declawing an animal from W.A.R will result in immediate repossession of said animal(s) I agree that I will never allow the declawing of any animal that I adopt from Windmill Animal Rescue.
- I acknowledge and agree that in the event I am unable or unwilling to care for or keep the pet(s) named in this agreement, I will not abandon, sell, give away, or in any manner dispose of said animal before contacting (W.A.R.)
- I acknowledge and agree that W.A.R. makes no claims or representations as to the temperament of animals available for adoption and that actions of animals are different from people and often unpredictable.
- I acknowledge and agree that W.A.R. is available for questions and referrals AND requests to be notified of any problems with the adoption before other action is taken.
- I acknowledge and agree that the laws of the area in which I reside will be complied with in regards to the licensing of the animal.
- I acknowledge and agree that W.A.R retains the right to check progress and compatibility, and that W.A.R or its representative shall have the right to remove the animal from premise if the W.A.R. representative is dissatisfied with the condition and/or treatment of said animal. Upon such demand, I agree to peacefully surrender the animal to the W.A.R representative.
- I acknowledge and agree I will provide excellent care to this animal by providing routine wellness checks and vaccinations as recommended by a licensed veterinarian, and that appropriate veterinary care will be promptly provided if said animal becomes sick or injured, and that kind treatment, water, proper nutrition, and shelter will be given daily
- I acknowledge and agree to the 10 DAY AGREEMENT: A separation from other pets in the home for 10 to 14 days is advised to avoid transfer of any medical issues. I understand that my pet(s) may appear healthy upon examination, only to show signs of illness shortly after. After 10 days, all medical issues are the responsibility of the adopter. If a health issue arises before the 10 day period has ended, I agree to take my animal(s) to a licensed veterinarian at my expense. Should the veterinarian find a medical problem the options are (1) Have a veterinarian treat the problem at that time at your expense (2) Contact W.A.R. for evaluation before proceeding with further treatment.
- I certify that the above information is true and understand that false information may result in the nullification of this application. I authorize W.A.R. to verify information with my veterinarian. W.A.R. reserves the right to refuse any adoption. This application and the information given herein remains the property of W.A.R. I affirm that my electronic signature was signed by myself with full consent.
